(Basic Policy and Initiatives for the Protection of Personal Information)
In order to protect the rights and interests of individuals, we believe that it is our social responsibility to appropriately handle and protect personal information in all situations where we provide services. In addition to complying with laws and regulations concerning personal information, we will clarify the following Privacy Policy and strengthen our internal system for the protection of personal information.

(Acquisition of Personal Information)
  1. The Corporation shall clearly state the purpose of use in advance when acquiring personal information of individuals (hereafter referred to as Customer), such as subscribers (pertains to individual members if the subscriber is a corporation) of the Kashiwa-no-ha Omics Gate service operated by the corporation and the services associated with it (hereafter referred to as Services).
  2. The Corporation shall acquire personal information through lawful and appropriate means.
  3. This Privacy Policy includes information on data, texts, software, music, audio, photographs, graphics, movies, messages, and other materials that Customers transmit to the Corporation (hereafter referred to as User Content Information), as well as information indicating how the online service provided by the corporation is used as collected by log files, cookies and Web beacon technology (including IP address, browser type, Internet service provider, reference page, OS, etc.; hereafter referred to as Access Log Information).
(Use of Personal Information)
  1. The Corporation can use personal information within the scope of the purpose of use, provided it is necessary for the performance of business only by persons with specific authority.
  2. The Corporation shall obtain personal information of the types listed in the left column of the following table for the purpose of use in the right column.

(Management of Personal Information)
  1. The Corporation shall appoint a personal information general manager and appoint a personal information manager in each department in charge of the management of personal information to implement appropriate security control measures to protect personal information.
  2. The Corporation will take necessary and appropriate safety measures against risks such as unauthorized access to personal information, loss, destruction, alteration, and omission of personal information.
  3. If personal information management is entrusted to a third party, the Corporation shall select a subcontractor who is tasked to properly handle personal information, enter into arrangements for matters concerning the safety management of personal information based on consignment agreements, and conduct necessary and appropriate supervision.

(Provision of Personal Information to a Third Party)
The Corporation shall not provide personal information to any third party without prior consent of the Customer. However, this provision shall not apply to justifiable grounds, such as when required in accordance with laws and regulations, or when permitted in accordance with laws and regulations, etc.

(Procedures for Disclosure, Correction, Suspension of Use, Deletion, etc. of Personal Information)
At the request of the Customer, the Corporation shall appropriately respond to the procedures for disclosure, correction, suspension of use, and deletion of personal information of the Customer in accordance with the following procedures prescribed by the Corporation.

Procedures for Disclosure, Correction, Suspension of Use, Deletion, etc. of Personal Information

(Request for Disclosure)
Through the following procedure, the Customer (or their representative) can request the Corporation to disclose personal information held by the Corporation regarding the Customer. However, please note that the Corporation may be unable to respond to all or part of the request in the following cases.

  • Cases where there is a risk of harm to the life, body, property, or other rights and interests of the person or a third party
  • Cases where there is a risk of causing a serious hindrance to the proper implementation of the business of the Corporation.
  • In the event of a violation of laws and regulations

(Request for Suspension of Use, Correction, etc.)
The Customer (or their representative) can request the Corporation to suspend the provision of personal information held by the Corporation to a third party. However, please note that the Corporation may be unable to respond to all or part of the request in the following cases.

②Identification document (one copy)
The following documents (one copy) confirm the identity of the Customer in order to prevent leakage of personal information to a third party:

  1. All health insurance cards
  2. All pension handbooks
  3. All welfare handbooks
  4. Driver's license
  5. Passport
  6. Alien registration card
  7. Copy of residence card
  8. Certification of information recorded on foreign resident registration file

*1, 2, 3, 4, 5, and 6 should be within the expiration date or currently valid. 7 and 8 should be presented within 6 months from the date of issue. The identification document must bear the name, address, and date of birth of the Customer.
*The above documents will be disposed by the Corporation after a certain period of time.

If requested by a representative, please send the following documents in addition to ① and ② above.

    A copy of any one of the documents listed in ②
  2. Following certificates (one copy) according to the relationship with the Customer
    Person with parental authority: Copy of family register
    Guardian of adult: Certificate of registered matters on the guardianship of an adult
    Voluntary Representative: Power of attorney and personal seal certificate
*Certificates must be presented within 6 months from the date of issue.

<Commissions>

  1. The following fees are required for disclosure requests when accepting application forms.
    500 yen per issue (including consumption tax)
    Registered mail or stamps worth 500 yen
  2. Requests for suspension of use, correction, etc. are free of charge.

(Information on Results)
After receipt, a notification form will be mailed to the Customer.
